Telemedicine Informed Consent

Please read this form carefully to understand how virtual consultations work, your rights, and the limitations of electronic medical care before giving your consent.

1. Definition and Nature of Telemedicine Services

• Service Facilitation: I understand that Penny Medical Tourism Agency Ltd provides the digital infrastructure and tools to connect me with independent, licensed healthcare professionals (‘Providers’).
• Provider Credentials: These Providers are independent contractors who are registered with the Health Professions Council of Zambia (HPCZ) or other applicable international medical regulatory bodies.
• Agency Limitation: I recognize that Penny Medical Tourism Agency Ltd does not provide medical care or clinical diagnoses directly. All clinical evaluations, prescriptions, and medical advice are exclusively delivered by the independent consulting Provider.

2. Benefits and Potential Risks

• Expected Benefits: Telemedicine enables remote access to medical evaluation, online pre-travel consultations, specialist opinions, and follow-up care without needing to travel initially.
• Inherent Technical and Clinical Risks: I acknowledge that hardware or connection disruptions may degrade sound or video quality; security protocols could potentially fail despite strict encryption standards; and a virtual appointment cannot replace a comprehensive, in-person physical assessment, which may limit the Provider’s capability to finalize a diagnosis.

3. Patient Rights and Privacy Obligations

• I reserve the right to withhold or withdraw my consent to telemedicine consultations at any stage without affecting my right to subsequent standard care or future travel packages.
• I have the right to request any non-medical tech support staff to leave the virtual room, and I may also request a family member or caregiver to assist me during the session.
• All medical files, audio-visual streams, and electronic communications handled during telemedicine sessions are subject to strict privacy and processing guidelines under company policy.

4. Emergency Situations

• I understand that telemedicine services provided through Penny Medical Tourism Agency Ltd are strictly for elective, scheduled pre-travel evaluations, secondary opinions, and follow-up appointments.
• CRITICAL: In case of an urgent or life-threatening clinical medical emergency, I will immediately contact local emergency services or present myself at the nearest physical hospital or emergency care clinic.

5. Informed Acknowledgment

• I certify that I have read this document, understand the specific limitations and risks of virtual consultations, and explicitly authorize the designated independent Provider to evaluate me.

Medical Travel Assistance Services – Patient Informed Consent and Service Agreement

Please review the following before giving your consent.

1. Scope of Services and Facilitator Role

• Role of Penny Medical Tourism Agency Ltd: I understand that Penny Medical Tourism Agency Ltd acts as a medical travel facilitator and telemedicine coordinator, connecting patients with independent partner hospitals, clinics, and licensed medical practitioners locally and abroad.
• Independent Healthcare Providers: I acknowledge that medical, surgical, and clinical treatments are performed independently by licensed physicians, surgeons, and healthcare facilities locally and abroad, not by Penny Medical Tourism Agency Ltd employees.

2. Medical Records and Information Sharing

• I hereby authorize Penny Medical Tourism Agency Ltd and its staff/coordinators to collect, review, store, and transmit my medical history, diagnostic test results, imaging, and consultation notes to overseas partner hospitals and doctors for evaluation.
• I understand that my data may be processed both locally and internationally in accordance with the regulatory framework and the Penny Medical Tourism Agency Terms of Service.

3. Acknowledgment of Risks in Medical Tourism

• Inherent Medical Risks: I understand that any medical, diagnostic, or surgical procedure carries inherent risks, including but not limited to infection, adverse drug reactions, complications, or failure to achieve desired outcomes.
• Travel and Post-Operative Risks: I acknowledge the risks associated with traveling post-procedure (such as deep vein thrombosis/blood clots during flights, climate changes, or language/cultural barriers).
• Direct Consent with Treating Doctor: I understand that a separate, detailed clinical consent form will be administered, explained, and signed directly with the overseas hospital and treating surgeon prior to any medical intervention.

4. Financial Responsibilities and Disclosures

• I acknowledge that Penny Medical Tourism Agency Ltd provides upfront pricing estimates based on information supplied by partner facilities, and that unforeseen clinical complications or requirement for extended hospital stays may incur additional costs payable directly to the hospital or service provider.
• I confirm that I am responsible for travel insurance, visa arrangements, and any out-of-pocket expenses not explicitly covered in my selected package.
